The Parties: This agreement is for services for the even described below, between the undersigned Client and Chocolate Velvet Fountains. Chocolate Velvet Fountains agrees to furnish services to the following Client and specifications:
1. Photo Booths:
OR USB Flash Drive
__Dippers @ $1.00 per Guest (Minimum 150 Guests).
Payment Terms: Total fee agreed upon by the client and Chocolate Velvet Fountains is $ ___________
A deposit is required of $200.00 or 25% of the fee agreed upon (whichever is greater). The balance is due no later than 14 days prior to the date of the event. Overtime is calculated in half hour increments for $75.00. Payment is due at time of request. This agreement can be canceled in writing only, by either the Client or Chocolate Velvet Fountains. If Chocolate Velvet Fountains initiates cancellation for any reason within or beyond its control, liability will be limited to returning with in 7 days all monies prepaid for services here within. Provisions: Pertaining to Photo Booths: Client understands that changing of paper and ink or other adjustments may be necessary. Client will provide attendants with dinner. Liability: Chocolate Velvet Fountains shall not be liable for any injury, loss or damage directly or consequentially arising out of use of inability to use any of its equipment.
